One of my pal left comments on my blog. Strange thing is, I have received the comment notification from his nick name in stead of 'Wordpress'. Although I haven't approved his comments yet, his comments are live on my blog.
How did it happen? Is it a security threat?

It's always possible.
How did it happen?
Hard to say. Check your discussion settings in your dashboard first, and then audit your users. Look for any accounts you know don't belong there. Then take it from there. But, this is the first thing I see that might be a tip off. Then again it could be totally unrelated.
