Have you tried to recreate the feedback page but giving it a default permalink rather than any custom you might have created?
I suggest re-create the form [again] and then see what happens.

I’ve tried giving it the /comments permalink and it works fine – the only thing is that we have printed leaflets with the /feedback link in a QR code, so I need to find a way to get this working with this link.
Have you also created a re-direct to your pages by changing the .htaccess file? As far I am aware, there are two steps for chnaging the Permalinks:
1) you go to the dashboard and Settings >> Permalinks >> Postname
2) change the redirects by changing the .htaccess.
It looks like you haven’t done step 2 so you need to do it to re-direct old pages. It is something like this:
RedirectMatch 301 ^/$ http://mytaxsite.co.uk/?p=$
Can you post a link to your page so that I can try to give you a redirect code for it.

Unfortunately I have tried this as well, and had checked it using a .htaccess Redirect generator just incase it was the wrong syntax. The site itself isn’t on the internet yet as I am developing it using MAMP. Once it’s ready I’ll post a link and see if a solution can be found.
Failing that I guess it will be plan B, and to create a feedback/index.php file which has a header redirect to another location within the site.
