Auto-populate with specific page data

Hello! I’m trying to figure out a solution to this website I’m working on (artiquesimports.com/wordpress) — Next to each item in the “single post” view for the various portfolio items, I’d like to add a contact form for each item that automatically populates with the title of the page. That way, when someone inquires about the price of a product, the owner sees which product the person was referring to specifically.

Would this be too difficult to figure out? Or should I try a different solution?
