[Plugin: VoucherPress] Changes to form for name and email for style reasons

Hi Christian,

I want to be able to have the entry point for the nema and email in a widget area as shown in this image

This says newsletter signup but we want it for your voucher. So the only change is the way the two entry fields are displayed and the button.

If it needs changes to the php,css and html would you do it for a price. Or can we do it just for our one site. The site in question is http://www.maharishiayurvedaspa.co.uk.
