[Plugin: WP Customer Reviews] Form on one page Testimonials on another?

Hi,
I would like to display the Form on one page and the Testimonials on another page.
I’ve tried adding the following to my CSS but it doesn’t seem to eliminate the Testimonials from showing up on the Form page.
#page-id-697 .hreview { display: none !important; }
Any suggestions?
