Audit trail for posts and pages with editor comments?

    We are using WP for a support and tutorial site of medical software. Quality and regulatory requirements are that we keep a track of every and any changes made to the site.

    I’m looking for a plugin that can keep log of all page and post editing, and allows users to add a comment to the log, aka why something was changed.

    Anyone with ideas, couldn’t find a plugin with my search skills..?
