  • I was hacked a while ago and wonder if this is something left over?

    Quite a possibility, but a quick scan of your site does not show signs of it. But you may note that such scans do not show some hacks, especially adware, which seems to be the case here.

    Problem is I have replaced all wordpress files and plugins so have no idea how this is getting there.

    Any ideas?

    You mean you did not install it? If so, it is an intrusion, similar to a hack.

    You may probably find it in header.php. If you do not find it it is just loaded when the site is up. Then it can be from any of the scripts, external links/ apps, plugins, and even your theme.

    Turns out that it is included in my theme – for some reason the theme author puts that url in his themes – which still leaves me with the issue of the weird message on log out!

    Then you need to change to some other theme. You are being exploited by it.
