[Plugin: Usernoise modal feedback / contact form] Form contents will not update

Thank you for your work on this plugin. I am using Genesis 1.8 with StudioPress Magazine child theme.

Plugin installed fine and out of all of the modal/popup plugins I tried, yours is the only one I was able to get to work.

The only problem I have is that the form will not update. I am using W3c Total Cache and WP-DB Manager; cleared all the caches and optimized the DB, but still nothing changes in the form–everything else updates fine.

Do I need to put some code into the Genesis Hook boxes? If so what code should I use?
