[Plugin: Forum Restrict] dashboard goes blank when i activate

    As soon as I activate this plugin, my wp-admin dashboard goes blank. the site itself stays up, and as soon as i change or delete the file via ftp it all comes back. i tried once, updated my version of filezilla, and tried again, same thing.

    this has happened with a number of, but not all other plugins i’ve installed recently, including other ones with a similar function to this one.

    help!

    update: i turned on wp_debug, and this is what it gives me that relates:
