[Plugin: Constant Contact for WordPress] plugin not working – no other plugins active – notices and

Hello all,
Constant Contact API Version 2.3.9 | WordPress 3.4.1.
I’ve disable all other plugins and have tested with Twenty Eleven.
I’ve also installed the plugin twice to make sure it wasn’t corrupted.

Here are the symptoms:
When activating the plugin (i have debugging on) I see this notice in the admin & front end:
Notice: A session had already been started – ignoring session_start() in /Applications/MAMP/htdocs/tr_wordpress/wp-includes/plugin.php on line 403

When trying to update any plugin settings i get the following:

I’ve also noticed the plugin admin formatting on this page is very off.
