[Plugin: Fast Secure Contact Form] Checkbox field is not configured properly in settings?

On the page with my contact form there is the following error displayed in red text:
Error: A checkbox field is not configured properly in settings.
I can’t seem to figure out what the problem is so I’m wondering if someone could help? Thanks.
