Custom Comments plugin

Hi, I’m having some trouble figuring out how to get the Custom Comments plugin to work – http://www.valhalla-fr.eu/index.php/2005/11/01/plugin-pour-wordpress-affichage-personnalise-des-commentaires/

My comments.php page is different to the example given and I’m not sure how or where exactly to insert the code.
