    I’m not sure what is causing that to happen, but I’m adding an option to specify the ‘destination’ URL directly in the next update, which should solve your problem. However…

    The form shortcode uses get_permalink() for the form action. I noticed on both of the forms you linked to, the form action was set to http://(...)/contact, instead of http://(...)/category/contact. On the first form, this gave a 404 error. The value of get_permalink() should not return a 404 error. So there is something going on between permalink generation and parsing.

    The new setting is in the form editor > Advanced > Behavior > Exact URL

    Thanks for that! All seems to be working well now. The form definitely didn’t like the ‘category’ part of the URL. But using SEO for WordPress by Yoast I stripped it out and now it is working perfectly.
