[Plugin: Jazzy Forms] Need a little help setting up simple take-out menu

I like your new plug-in- very minimalist. So minimalist there aren’t that many instructions 😉 I’d really like your help- I’m just trying to set up a simple form for a restaurant’s take-out menu.

Here’s what I have so far: http://i.imgur.com/EIPpc.jpg. Basically just 3 items with an text field next to each so that the user can enter the quantity they want. Next, a button so they can see the total of their order, then finally a submit button that sends the restaurant an email with the order info.

Questions:
Where do I enter the prices?
What should my formula be?
How do I specify the text field width?
