User submitted content transmitted to moderation queue for a blog post plugin?

Is there a WP plugin for user-submitted content that sits in a moderation queue until a moderator reviews and approves it for publishing it as a blog post, not a blog comment? Or other way to do this?
I don’t want to use a multiuser version of wordpress or have people sign in for limited access to wordpress and post since in this case they’d only be posting one time and that’s it.
What I want is for people to be able to submit their full name or initials, their city and their story (see below) and have it go to a moderation queue, than I and others review it and turn it into a blog post with the city being the category. Ideally I’d like this as automated as possible. Then if people want they can comment on individual posts.
Purpose: This is for the purpose of setting up a blog to allow people with ADHD share their stories of what life was before getting diagnosed with ADHD, and after diagnosis and treatment to give examples of real life stories to the public, media and govt in the hopes of allowing more people who need diagnosis to get it. A lack of trained professionals is a big problem in this area and hopefully this is one way that will change that.
