Problem with WordPress permalinks & Facebook share

Recently, my site has been experiencing weird inconsistencies when sharing to Facebook.

Sometimes (not all the time), Facebook only picks up the URL, not the description or the title, leading to FB posts that look like this:

Other times, it appears to work fine right off the bat:

The only way to fix this the problem seems to be to change the WordPress permalink AFTER the post has been published. Using a custom permalink before a post is published does not yield consistent results, but if I change the permalink after a post is published, it always fixes the problem.

Any thoughts on why this might be occurring?

I have tried disabling all plugins, and even using different themes (like 2011), and it continues to do it. It also does it regardless of whether I use default permalinks or the date & name permalinks.
