[Plugin: MediaElement.js – HTML5 Video & Audio Player] Audio plays but no video

I’ve had a couple people respond that they can hear the audio of the mp4 playing but do not see the video. This was a mp4 encoded using x264. It plays fine for me on Chrome for Mac. Any one have any ideas?
