[Plugin: WordPress SEO by Yoast] Duplicate website title on post title

I have installed the SEO Yoast plugin and it it generating the site title next to my post title on every page.

I have looked around and can’t find any help on this matter. I have tried the Force Rewrite titles box but nothing happened. I have also already tried to change the header.php with: <title><?php wp_title(”); ?></title> and again nothing happened. Currently the title section in my header.php looks like this: http://pastebin.com/LFXyqaJJ
