Fee calculator form?

    Basically I am looking for a basic calculator form where it would have 3-4 text box. The first text box would be where a user would enter a value. The 2nd text box would show the value (2.9% of the entered value$0.30) and the 3rd textbox would show the sum of the first and second text box.

    Does anyone have any suggestions on which widget would do that?
