[Plugin: Fast Secure Contact Form] Adding the email form manually to sidebar

I am trying to add the form manually to my sidebar but there is nowhere to explain how to add it manually.

I have tried copying the html code from a page I have embedded the form into but the captcha will not refresh.

My website is Protocol-Finance and I am trying to replace the email us that is on the contact or about us page sidebar. The website is still not complete so most links will not work besides the about us and contact us.

Is there any code to add the form manually the is the whole form right up to the captcha?
