I need a user content upload form

I am starting a blog for Diabetic foods/diets. I need to have a separate form other than the comments form for subscribers to upload recipes, inc images if they wish.
I would want these to go to another email address for me to moderate them.
As usual all the near misses I have searched out, go from ABC to rocket surgery in two sentences. I am a beginner.
