• Even in the latest version of the plugin, I cannot remove the first and last names from a form I designed.

The widget apparently uses a form that is not editable in the form designer. Is this correct? What is the form id of this form so that I can call it via the shortcode?

You can see me trying to use the forms on a site I’m building at http://www.patchadamsclinic.org.

Maybe you can quickly see what’s happening on my site and advise how to resolve the issues.

Maybe all of this has to do with the Awake theme we purchased for this web site. If so, we’ll need a different solution than your plugin.

    Thank you, jim

You need to create a new form in the form designer, save it and it will show up in the Form Designer widget in the dropdown menu as an option.

Right now, the form ID is 0.

I can create forms using the form designer and have created 2. However, I don’t want the first and last name to appear on these forms and cannot eliminate them. I uncheck the 2 boxes and save and they reappear. I would also like the Constant Contact list of available newsletters appear in these forms and don’t see any way to add that.

So, it only seems that the default form meets my needs (except that I can’t customize it). And this is fine right now. Can you please point me to documentation that will tell me how to get the default form to display in a non-widget area on my site? The home page on the site is actually a page.

You did reply to another of my posts about placement and said to use:
    [constantcontactapi formid=”YOUR_FORM_NUMBER_HERE”]

I’ve tried using 0 which shows me one form I made. 1 shows me the second form I made. I can’t pull up the default form.

Thank you for your help, jim

The “default form” id is -1, but I am not sure that will work; as the default form does not really exist? Couldn’t you simply save the default form and use that form ID?

You are right in that -1 does not work. How can I save the default form when there is no way to access via the form designer in order to save it. I created 2 other forms, and would use one of them but I can’t eliminate the first and last name as in the default form.
