Permalinks Not Working…Urgent…Will Pay

I have a custom theme with a a few custom post types. When I set the permalink structure to default everything works fine. However, when I set the permalink structure to /%post_id%/%postname%/ and hit the save button, the custom post types return a 404.
Now get this, when I go and re-save the custom permalink structure, the custom post types begin to work fine, BUT now the regular pages return a 404. I NEED help immediately on this and will pay for someone who is really good with PHP to go in and fix it.
