I get 404 error when try to comment post with mTouch Quiz

First, thanks for great plug-in!

But from one of the last update I have a problem.
When I try to post a comment on post with mTouch Quiz – 404 error appear.
For example this page – http://3dyuriki.com/2011/02/11/test-na-znanie-3ds-max-1/ Just try to post comment and you will see the problem.

If you can help me – I’ll be very thank you.
