TFT Titles == WTF Titles? How to get this working???

Seriously, can anyone who writes a plugin make some instructions that make sense?? I really want to use a headline image replacement program (running 3.04), I know PHP reasonably well, but I still can’t figure out WHERE to put the code for this.
Sooooo frustrating!
Anyone who has this working give some insight? Even the supposedly clarifying instructions don’t really make much sense, honestly.
All I need to know is WHERE to put the supposedly “obvious” code, and how to make it work. So hard?
