Problem with Gamez WP-Poll for 1.5.2WP version

Hello. Today I upgraded my blog from 1.5.1.2 (or sth like that) to the latest version 1.5.2 and my WP-Poll I took from GAMEZ site, most of you have it and know it, doesn’t works. I get the following error. How can I fix it? Thanks!
