Weird, random 404 error with permalinks

    On our site (http://www.lutheranworld.org), we have set up permalinks as “/index.php/%postname%.html”. Strange, but it works.

    But, sometimes, we experience 404 on newly created posts and pages that seem to be due to the permalink.

    It is rare and usually takes not much more than a bit of fiddling with the permalink to solve the issue, but it is highly annoying.

    Anybody experience this? Any solution?
