[Plugin: Identi.ca Tools] Linking hashtags to identi.ca?

I’d love to see hashtags, both in posts and the sidebar (via aktt_sidebar_tweets()), be linked to http://identi.ca/tag/foo — or some groovy tagsearch site or other.
