I’m not sure I follow your exact thinking, but if you are after a
custom permalink structure that includes /%category%/%postname%/ you would
set those values here: Settings Permalinks SubPanel
And here is a quick reference for Using “Pretty” permalinks
In your WP admin panel, In the Settings → Permalinks panel, you can choose one of the “common” structures or enter your own in the “Custom structure” field using the structure tags.
%category%-%postname%
NOTE: “Starting Permalinks with %category% is strongly not recommended for performance reasons.”
I use: /%post_id%-%postname%

also, if it’s doing taht annoying +2 +3 thing, and you don’t have other posts with the same name, its because you have versions of that post in the trash. Empty your trash from the edit posts screen and that will stop
