    Using version 4.3.7 of this plugin.
    I should probably consider myself lucky – because I’m NOT getting locked out. But the trouble is, nothing is triggering a lockout.
    I have set the plugin for 404 Detection Enabled, and lockout for 15 minutes after 10 errors in 5 minutes.
    But I am unable to trigger a lockout. Hit a nonexistent URL on the site 15 times. The plugin dutifully logs the attempt, but does not lock out. I am not hitting one of the exceptions that you can configure, before anyone asks.
    I am not allowing the plugin to write to .htaccess or config.php. But this is not the issue, as there are no new rules in either of those files for me to add.
    Also, email notifications do not trigger.
    Any thoughts? 404 detection is one of the things I rely on this plugin for.
