plugin(s) that allow visitor to upload photo with form for photo info & consent

    I am looking for a plug-in that allows a user to upload an image to the site and input information about themselves and consent to the image being used on the site through the front end. I am willing to update the version of WP if it is necessary.

    Same HERE !!!… nothing but TDO Mini forms in plugins … and its not user friendly !!! I wonder why nobody made File (Images) Upload plugin (widget) for WP Frontend…

    I have found one more … but bad one .. it is only for web design qoutes …plugin is called ImageUploader 🙁

    Use Advanced Text Widget to add it like a widget to your side bar if you want too …
