First off you can view the form I'm building here. Now I have filled this out and sent it to myself, but this is all I received im my email...

I also can't figure out how to change the names of these fields so that when they arrive in my email I can see them. Right now all I'm getting is "Message body" and the subject line of the email is just "your-subject".
Any help with this would be appreciated.

Also, you can learn how it works by using "Generate Tag" button next to the Form field.
I've already been to that page you linked - it comes with the documentation. Also, the generate tag drop down is no big help. This doesn't answer any of the questions I had in my original post.
Anyway, doesn't matter. I deleted that plugin and got a much better one with better support.
