Need help with wbExam plugin

I’ve installed wbExam plugin, made my page template, and made up my question file and made the custom key. But the only thing that shows up on my page is <form action=”” method=”post”>
<input type=”hidden” value=”1″ name=”QNR”/>
<input type=”hidden” value=”0″ name=”SCR”/>

<input type=”submit” value=”” name=”.submit”/>
</form> (from source code)
Which shows up as about a quarter of an inch worth of submit button but nothing more.

Anyone out there using this plugin with words of advice?
