TDO Mini Forms – Append Item on select menu not working

I just activated TDO Mini Forms. I am trying to create a drop-down list. When I use 1 row for a drop-down list and try to add items to the list with “Append Item”, they don’t appear on the form. When I try to add them again, I get a message that says they are already there. I don’t know what I’m doing wrong. I sure wish that there was some documentation with this plug-in because I’m lost.
