   How do I remove the questions address, city, state, and zip? Thanks!

   I have modified this addon to not include address, city, state and zip by default. It only asks for first and last name and email. Of course, the above fields can be added by adding user defined questions. If you want me to send you the new, send me an email at : mboisso at zepto.ca.

   Can you point me to where this can be modified in the code? It would be great to see this as part of the default install. Depending on the environment, it may not be necessary to collect more than name and email. Thanks -
