   I need to set up a form to capture membership details in a database. It needs fields like text, numeric, multi-choice, multi-level (e.g. "red","green","blue" multichoice, plus "other" to a 2nd level of text, for example, "please state"), so must be customisable.

   Does anyone know of such a plug-in?
