Need help turning layout into a template

I have the layout all coded and ready, but I have NO idea how to turn it into a template. I’ve read various tutorials, read the WordPress site, and I’m still stuck and don’t even know how to break up my layout into the template pieces.

If anyone could help me turn this into a template, I would be eternally grateful or any help would be nice as well.
