poll or survey plugin and counting answers

I need to create a survey ( 15 questions) answers are always
yes or no ( buttons or check boxes ?)
I need to show at the bottom of survey form
your result : xxx yes YYY no
if you have between 0 and 6 yes your profile is : 1
if you have between 7 and 12 yes your profile is :2
if you have more than 8 yes your profile is : 3
Anybody knows if a plugin can achieve this if no how to achieve this
