I'm running 0.63 version of FormBuilder and seem to get the following error regardless of the content loaded into the form, "Oops. You have already submitted this form data once."
Example pages on my site are:

What version of WordPress are you running? Also, are there any other custom modifications I should know about?

Sorry, I am running 2.6.5. The only other customization I can think of is Max Banner Ads Pro plug-in and iThemes Flexx Theme.

Wow. I'm really not sure what would be causing this problem. I suppose it's possible that either a) your host doesn't support the $_POST variable in PHP, or b) your host doesn't have MD5 support built into PHP.
Other than that, I'm really not sure. Are you able to find out what version of PHP you're running?

Sorry for the delayed response. My host is BlueHost and my version of PHP is 5.2.8. I'll contact them today to see about the $_POST variable.

I also have the same problem
I use Godaddy for hosting
I have WP version 2.7.1
I am using version 0.80 of the form builder
do you have any idea on how i can fix this problem

Do you know what version of PHP you're using, and whether you are on a Linux host or a Windows host?
