   v3 puts a duplicate post title for example Home > Some Page > Category > Some Page

   The most common cause of this problem is a permalink configuration that needs a refresh (I have not been able to reproduce it on my test bed server). Rebuilding your permalinks should fix the problem. If you are using a static front page, you may need to reset the page on front setting as well.

   Someone else had this problem (again), and gave me a little information about their setup. What is probably happening is your static front page setup is not correct. In addition to specifying the front page you must specify the posts page (under Settings > Reading). I can verify that not having a posts page specified causes a problem (both from a logical standpoint and from testing). The plugin makes the assumption that if you are going to use blog posts in a static front page setup you need to have a main page for them.
