I see the same problem. I had to roll back to 3.9.2 since Collapsing Categories is crucial for me.

The widget has disappeared for me too
The symptoms I think all of us are seeing (in WP 4.0 only) are that after installing the widget (yes, I tried deleting and then reinstalling the widget) it simply does not appear listed anywhere as a widget, not even as an inactive widget, and not even as one that is available to activate anywhere.
My installation of Collapsing Categories had silently survived a number of previous WP upgrades (from 3.8 through 3.9.2), so this behavior is unique to the ->4.0 upgrade step.
Does anyone have different symptoms to report?
I’ve got the same symptoms. Upgraded to 4.0 and “poof”. Deactivate and reactivated, no avail.
