Loaded Media not showing – all images on front end missing

Somewhere in the last 2 updates the media file has stopped showing on the admin site. When I try to access the media it shows empty so I cannot add any media files that are still in the media files (In media and via posts, pages, featured images etc). As a result all linked media, and the links are gone. The only media showing is the logos and sliders in the Rev slider plugin.

I have tried deactivating all plugins and swapped to the default WP theme but still the same. I tried the standard htaccess file and also uploading and replacing the wp-includes and wp-admin files form a clean wp download.

I have also tried rolling back to earlier backup buddy versions but still the same issues?
