I activated contact form already again.
I didn’t use many other modules on this site but Google profile & site verification were fine.
I don’t know why it happened.

Hi Brandon,
I just realized that on my other website, update of the Jetpack deactivated Custom CSS module and blog subscription that I used in the footer.
here is my other website:

Both website on same server but I use different themes and different plugins.

Can you please use Jetpack’s Debugger to send us some more information about your site?
To do so, follow these steps:
1) Go to the Jetpack page in your dashboard.
2) Click on the Debug link in the footer.
3) Click the link that says “click here to contact Jetpack support.”
4) Fill in the description box with a link to this thread and your name and email address.
5) Click the “Contact Support” button.
