[Plugin: Post from Site] How to remove Post Format option and . . .

I am having two problems with the Post from Site Plugin by Ryelle.
My goal is to create a photo submit site using the plugin and my goal is to keep things as simple as possible.

1. I want to remove the option to chose different post formats which are [image] and [gallery]. It’s also weird that image options gets doubled. Please see image from link below:

2. After my internet browser window reaches a certain width, half of the popup window from the Post from Site plugin moves off screen and can’t close it unless you refresh screen. image below: I wish to make it open centered on the page. A background gray-out or lightbox situation would be even better.
