  • This post is meant for reference if anyone else experiences this issue and finds it on Google, I hope this helps. I have Ninja Forms Pro with a few of the Ninja extensions. Using the Multi-Part Form plugin all the fields on the first page were being posted to the database as empty fields, even though First Name / Last Name and a few other fields were REQUIRED. Below is a ticket I sent to Ninja Forms team.

    A very strange thing has occurred about 5 days ago.

    Observation
    All submissions are coming in blank accept for a few fields. After looking at a recent CSV file, I have determined that all fields on the first page of a Multi-Part Forums enabled form were showing blank. Everything Page 2 and beyond shows up in the CSV file.

    Experiment/Work Around Solution
    After determining that it was related to the Multi-Part Forms Plugin; I unchecked “Enable Multi-Part Form” and “Disable Progress Bar” – saved the form. Then ran a test. Submissions received the first and second page of information. CSV download now has first page of information. I went ahead and reenabled Multi-Part Form on the effected forms. No more issues.

    What happened?
    I am not entirely sure what happened 5 days ago. We currently are running this server on AWS Elastic Beanstalk. The git deployment of this instance hasn’t been updated since 3.9.1 release, which was about a month ago. I went ahead and updated all the plugins while looking at this specific issue. I am unable to determine while the server wasn’t seeing the first page of fields for the Multi-Part Form plugin. Perhaps the variables weren’t being properly passed through POST?
