Help setting up – won't display categories

Perhaps I’m missing something in the set up because it seems to be working but is telling me that “There are no categories to display”. It looks easy to set up but I don’t know what I’m doing wrong. At one point in fiddling with it I saw one category in the list but it wasn’t one that I wanted. Please help.
