having problem with child theme

After following the instructions here http://codex.wordpress.org/Child_Themes , I created the theme But when I went to appearance in the theme zone I can’t find the child theme to activate.
Please teach me if am wrong on how to activate it.

Can you link your website & tell us what is your Child Theme folder name?

for the theme name I copied everything at the top of the style in the parent folder
One thing is missing from style.css header why it doesn’t show up in theme preview:

You should understand that screenshot in theme preview will be blank because there is no screenshot if you haven’t uploaded it manually.
And make sure that child theme folder is called
