Voting up/down or thumbs up/down plugin needed!

I’m trying to replace gd star ratings and am having the hardest time finding a simple plugin (with shortcode that can be entered anywhere) that visitors can vote yes/no, up/down or thumbs up/down. Basically I want to ask a yes or no question and have them vote accordingly.

I’ve been looking for weeks, but can’t find one! I’ve even looked into paid plugins, but am having zero luck. HELP!!!
