I have tested it on a new installation (localhost). Above each box an empty field is created. (wp 3.9)
What theme are you using? Can you switch to buddypress default theme and see if the error is still there?
I have tested it on TWO different pages:
1. rtpanel (wp 3.9)
2. Twenty Fourteen (wp 3.9)(fresh installation only buddypress and Xprofile)
I think it’s an issue of wp 3.9
I have know tested it with
buddypress 1.9.2
and
XProfile 1.5.8.4
Everything works fine.
After an upgrade on buddypress on 2.0 above the checkbox acceptance an additional empty fields appears.

There is also an additional field above the age and it is not possibel to change the age. I do not know what happens.

This seems to happen when you use a theme which is not prepared for buddypress. In my test site, I’ve installed buddypress default theme and it’s working well.
I’m in contact with buddypress developers to solve this “bug”. You can follow the thread here. If they say me this is not a bug, then I will need to update my plugin and probably rewrite many things so it can take a while.
To solve this now, you need to install bp default theme or a buddypress theme. If you can wait, follow the thread and we will see how it goes.
