Editable Fields to include in message body

I really like the idea of the messaging coming from a page instead of the dashboard. I would like to be able to define some fields that are sent in the message.

In addition to the standard To, From, Subject, Body, i would like to be able to set 3 or 4 fields that could be included in the body of the message, but i define, so people know they are required information. Is that possible with your plugin?
